Error reason code

Loupe

A specific, reportable statement of why something failed, attached to the failure itself.

What is an error reason code?

An error reason code is a specific identifier for why a particular check failed, attached to the failure itself. It distinguishes between a missing document and an illegible one, where a generic validation message treats them alike. Because the code is structured, failures can be counted by cause across a whole cycle.

Also called: Validation reason code

Why a generic failure message costs twice

An applicant who reads that their submission could not be processed calls the helpline, where the person answering knows no more than they do. That exchange is the largest avoidable cost in regulated intake, existing only because the failure was recorded as a message instead of a cause. A reason code attached to the failure tells the applicant what to correct and tells the organisation which checks are failing most. Those are the same fact, reported to two audiences.

  • A code separates causes that a single message collapses together.
  • Counting failures by cause identifies which form section needs redesign.
  • The applicant receives an instruction rather than a statement of refusal.
  • A stable code survives rewording of the message shown on screen.
  • Support staff can resolve a call without reproducing the submission.

How an error reason code works

  • Every check declares the codes it can emit before it reaches production.
  • Each code carries an applicant-facing message and an internal description.
  • The code is stored alongside the failing field and the time of failure.
  • Messages are translated per code so wording stays consistent across languages.
  • Codes are reported by frequency against the section that produced them.
  • A retired code is kept resolvable so historical records remain readable.

How an error reason code is evaluated

An error reason code is judged by whether an applicant can act on it unaided. Measure the following across recorded failures:

  • proportion of failures carrying a specific rather than generic code
  • share of applicants who correct the issue without contacting support
  • helpline volume per thousand failures, reported by code
  • distribution of codes across form sections
  • count of distinct causes collapsed into one code

Limitations, risks and common failure modes

  • A catch-all code absorbs most failures and conceals the real pattern.
  • Codes proliferate until nobody can report on them usefully.
  • An internal code leaks into applicant-facing text and means nothing to them.
  • Reporting is performed on messages rather than codes, so rewording resets the series.
